The original 'From The Inside' came in a die-cut sleeve with maching inner sleeve. The front cover opened in the center forming a "triptych" to reveal a scene inside a hospital. A door could be opened to reveal Alice in 'The Quiet Room' which was on the inner sleeve. The back cover featured doors that also opened to reveal the inmates escaping.

Pressings exist from at least Capital Records Pressing Plants in Los Angeles ("LW" on matrix/runout), Winchester ("WW" on matrix/runout) and Jacksonville "JW" on matrix/runout).
